| ¡¡¡¡Myelodysplastic syndrome (MDS) is considered as a kind of abnormality in the hematopoietic function resulted from the clonal mutations in early-stages hematopoietic stem cells when they are injured. MDS is a kind of clonal disease for it is developed from the pathological changes of one stem cell. It is a kind of common refractory disease in the apartment of hematology. Its major symptoms include decrease in blood cells, over proliferation of myeloid cells and morphological abnormalities of mature and immature cells in peripheral blood. The morphological abnormalities of mature and immature cells in peripheral blood differ from both normal cell proliferation and leukemic cell proliferation, and is a kind of pathological proliferation in between. This kind of disease often develops into leukemia and is thus often considered as the prophase of leukemia by some people. However, some patients were improved after treatment and did not develop into leukemia and some patients died of infections, hemorrhages or other causes and did not show any symptoms of leukemia all through their courses of the disease. So not all cases of MDS finally develop into leukemia. Therefore MDS is not the prophase of leukemia. The prophase of leukemia is the retrospective name for the disease course before leukemic symptoms have been certified. Apparent symptoms of MDS can be clinically observed in only a minor part of all the patients suffered from acute leukemia. Characteristic abnormalities in chromosomes can be observed in about 50% of the patients suffered from MDS. The progress of MDS and whether it would develop into acute leukemia depend heavily on the type and number of the activated oncogens in the cells. MDS can be divided into primary MDS and secondary MDS. Most of the cases are primary MDS. Its causes are still not clear. It often occurs in the middle-aged and the elderly males. About 70% of the patients are 50 years old or older.
It rarely occurs in children but its incidence in youngsters was increasing in recent years. Secondary MDS accounts for only a minor part of all the cases and occurs in patients with a history of exposure to carceinogenic substances, alkylating agents or radioactive agents. |
| The Clinical Symptoms of MDS |
| ¡¡¡¡Most of the onsets of MDS are hidden and abrupt. It occurs mostly in the middle-aged and the elderly males. About 70% of the patients are 50 years old or older. It rarely occurs in children but its incidence in youngsters was increasing in recent years. The first symptom of MDS is not characteristic and some patients experienced no apparent subjective |

| ¡¡¡¡symptoms. Most of the patients suffered from dizziness, short of strength, epigastric discomfort and arthralgia. Many of the cases started from anemia, which can be considered as the first symptom in diagnosis. This kind of anemia often lasts for several months to several years. About 20%-60% of the cases were accompanied with hemorrhagic diathesis of different severities, including dermal petechiae, gingival bleeding or epistaxis. Alimentary canal or cerebral hemorrhages may be observed in serious cases. This kind of hemorrhage is associated with thrombocytopenia. In some cases the patients were also suffering from malfunctions of thrombocytes. About half of the patients experienced fever during the course of their MDS. This kind of fever is associated with infections, mostly infections in respiratory tract. Other kinds of infections include septecemia, perianal infections and infections in perineum. The type of this kind of fever is uncertain. Infections or hemorrhages were the major causes of death in those cases that did not develop into acute leukemia. There may be moderate or mild hepatomegaly
or splenomegaly in some cases. Painless enlargements of lymph nodes occurred in a small number of patients. Some exceptional patients experienced tenderness of the sternom. |
